GOLDEN VALLEY, Minn. - KARE 11 has teamed up with The Convenings Project – a joint effort of Honoring Choices Minnesota and the Bruce Kramer Collaborative. Over the next several weeks, on air and online, KARE 11 will be featuring very personal stories to help you better understand the importance of advance-care planning.

Honoring Choices Minnesota is a collaborative, community-based Advance Care Planning initiative led by Twin Cities Medical Society through its Foundation. The medical society serves as the convener and coordinator, utilizing its unique access to health care organizations and medical professionals.

Beginning in the Twin Cities metropolitan area and spreading across the state, Honoring Choices Minnesota’s goal is to spur family conversations about future health care preferences and to assist health care organizations and community partners with the installation of a comprehensive advance care planning program.

The Convenings builds on two significant initiatives: Honoring Choices Minnesota and an award-winning series of broadcasts and book by journalist Cathy Wurzer that chronicled the life of Dr. Bruce Kramer. Those conversations led to the creation of the Bruce Kramer Collaborative.

Dr. Bruce Kramer and Cathy Wurzer collaborated to write a book about Dr. Kramer's experiences called "We Know How This Ends: Living While Dying."
